Description

PHYSICAL LOCATION:

Lubbock, Texas


MAJOR DUTIES AND RESPONSIBILITIES:

 

 Reviews in a timely manner the patient's pre-admission documentation

 Conducts family and patient interviews during intake process.

 Assures that IPP's, case management notes, and any other relevant documentation are filed in the

patient's medical record.

 Writes weekly contact notes and files in the patient's medical record

 Attends all treatment meetings concerning patients on his/her caseload.

 Serves as chairperson for IPP's, family conferences, discharge IPP's, and special meetings/conferences.

 Performs advocacy services for the patient

 Discusses plans, goals, and the status of the individual being served, with the patient and significant others to ensure patient involvement and family education on an ongoing basis.

 Provides a copy of the Individual Program Plan (IPP) to the patient or by request of significant others after proper releases are obtained.

 Explains long term goals of the Individual Program Plan (IPP) to the patient in conjunction with the licensed therapist.

 Assists with scheduling appointments such as SSDI, Medicare, Home Evaluations.

 Assists the patient with scheduling outside evaluations as deemed appropriate by the treatment team.

 Assists with process of arranging outside pursuits in coordination with the program staff, such as classes at local colleges and church, etc.

 Explains the amount of monies needed to sufficiently cover planned activities, medication needs or other needs to the individual, family, and funding source; may coordinate the receipt of these monies, when needed.

 Explains the rules and regulations to the family and the rights of the individual being served.

 Seeks advice from outside referral sources to intervene in special problems that may arise.

 Assists the individual and family regarding travel arrangements for weekends, holidays, and emergencies.

 Coordinates and assists with development of discharge plans for the individual before graduation.

 Provides appropriate referral information in coordination with the Follow-up Specialist to discharge community.


 Case manager will be available after hours as needed.

 Perform assigned duties in accordance with Moody Neuro organizational policy, CARF and other accrediting bodies.

 Perform other duties as assigned.


Minimum Qualifications

Required Knowledge, Skills and Abilities:

 

 Knowledge of counseling techniques and conflict resolution practices and the ability to implement them therapeutically.

 Demonstrate good organizational skills.

 Knowledge of the multiple and complex needs of the brain injured population.

 Knowledge of and ability to document according to facility standards.

 Excellent communication skills.

 Ability to work as a team member and facilitate the efforts of the treatment team.

 Ability to participate in the successful implementation of overall program goals.

 Ability to cope with stressful situations.

 Knowledge of licensing and accreditation standards (i.e., CARF, TRC, Medicare/Medicaid).


Communication and Interpersonal Skills:

 

 Establish and maintain courteous, cooperative service-oriented relationships with patients, the public, and other members of the health care team. Utilize courtesy, compassion, kindness, and honesty while interacting with the public, patients, and all facility personnel.

 Communicate with the appropriate supervisor, and other staff members regarding patient care issues in a professional manner.

 Identify problems/concerns, inform appropriate persons in a timely manner, and participate in a positive approach to the resolution.

Education:

Bachelor's Degree in rehabilitation, psychology, social science or accepted field from an accredited college or university. Master's preferred.

Experience:

Prefer two years' experience in Case Management Services to one or more disabled populations and or two

years' experience working with persons with disabilities in a rehabilitation setting.
